Red Card Verification Form (To comply with district requirements for tracking red card suspensions, the red card verification form is required. You should print out at least one copy so if a player or coach gets a red card you can start the verification process. This will be important if you are at a tournament where your player sits out a game or games before the red card even gets processed.)
