Micro Soccer

1. Completed registration form 1601. Please list the player name as it appears on his/her birth certificate. Be sure and sign the registration form. Registration forms will not be complete until the signature is obtained.
2. Mail completed registration form with a check payable to WVYSL. Mail to: WVYSL PO Box 10087 San Jose, CA 95157-1087
3. The fee for Micro Soccer is $80 per player.

U8 and Up

1. Completed registration form 1601. Please list the player name as it appears on his/her birth certificate. Be sure and sign the registration form. Registration forms will not be complete until the signature is obtained.
2. Copy of the player’s birth certificate or other government-stamped document showing proof of age (such as a passport). Hospital and religious certificates are not acceptable. If the birth certificate is in a foreign language and if you do not have a passport for the child, form 1627 Foreign Document Translation Form must be attached.
3. Player Photo. The photos are used on the player pass card and must meet the District Registrar’s criteria. The photo should be a 1″ x 1″ square of the players head. It cannot be a side view. No hats or sunglasses allowed. No Polaroid photos are allowed.

5. The fee is $90 per player for U8 and $110 for U10 and up.

Refund

• Full refund ONLY if child cannot be placed on a team.
• If a player has been placed to a team, but have not played a game, the registration fee minus $30.00 will be refunded.
• No refunds at all after the first game.
• Requests for refund must be made to the league in writing.
